PREPARING FOR YOUR UPCOMING APPOINTMENT

• Please note you have the current address.
• Please search the address and directions the day before in preparation for your visit.
• Make allowances for traffic, construction and weather
• Don’t forget any paperwork you think would be helpful for your appointment or specific paperwork, videos etc. your Doctor may have requested of you.
• Make sure you have a valid health card or refugee documentation if applicable
• Please arrive 10 minutes earlier than the set appointment time to have you ready for the physician at said appointment time.
• Babies take a while to get ready and triaged, and this is why we ask for early arrival and check in.
• Allow time for parking and getting onto the elevator.
• If you are running late, please call ahead to make sure your appointment won’t be cancelled and rescheduled. A no show fee may be applied.
• 10–15 minutes late may result in cancellation.
• Until further notice, only one Parent is allowed with the patient.

APPOINTMENT CANCELLATION

• Call 48 hours ahead of time to cancel appointments.
• If you have an appointment on Monday, you must call by Friday to cancel, as we are closed on weekends.
• If you are a first time Consultation visiting our physician for the first time you will be charged a fee for not showing to an appointment, this includes missed phone appointments. This is because we send our email/text reminders in addition to phone call reminders, and the Patient should be quite aware. By cancelling ahead of time, we can then accommodate another child/patient in need of an appointment.

REQUESTING APPOINTMENTS AND EXPECTATIONS

• Consultation first time appointments are usually accommodated within weeks. There is variability with Dr Margolis as he has limited clinic days per month
• Follow-ups should be arranged well ahead of time to ensure you get the date and time you prefer, and is best set up before leaving the clinic. This should also prevent the last minute calls for medication refills.
• Urgent/sick calls coming will be triaged, and the more urgent ones will be put in that day. Less urgent calls may be booked as a regular appointment, which sometimes can take days, depending on the physician. If your call was not accommodated, and you feel it is Urgent, please go to your nearest Urgent care, Emergency room or call 911.

WHAT TO EXPECT ON YOUR APPOINTMENT DAY

• We strive to be on time and efficient. Sometimes, due to other late arrivals or complicated patients, your physician may run behind. Please be patient. We usually strive to not run more than 15 minutes behind. Early, timely arrivals of patients helps the office run efficiently.
• Please be sure to check out on your way out and book for follow-ups before leaving, preferable to calling in at a later date.
• We mutually expect respect and cooperation from each other. If you have a disagreement with any of us, please settle the differences in a calm, humane manner.
• We are here to help your Child first and foremost.
• Rude or abusive behavior to staff in person or on reviews will be grounds for dismissal from the clinic. If you have a complaint that cannot be settled at the front, please request the Manager to contact you.

CLINIC OBSERVANCES DUE TO COVID, MEASLES AND OTHER POTENTIAL OUTBREAKS

• If you or a household member is sick and/or has a fever, call ahead to cancel
• Only one parent allowed per child
• If you or your child or both are sick, please contact us ahead of time and we can convert to a virtual appointment. Please keep in mind that there are only limited times that OHIP will allow for virtual visits.

FORM/LETTER REQUESTS AND MEDICATION REFILL REQUESTS

• Disability tax forms will be done usually within 6 weeks of drop off.
• Letter requests for school, daycare etc. should be requested ahead of the day needed.
• Med refill requests are expected 1 week ahead. You should keep an eye on when your child’s medication is running low. Your physician may or may not be there to do a refill that comes in urgently from a parent the day before the child runs out. This is unreasonable.

ONTARIO MEDICAL ASSOCIATION ALLOWED FEES

• Please note OMA has delineated fees for Physicians to charge for Letters, DTC forms, missed appointments, faxed medication refills without a visit, medical records transfer fees and more.
• Please view their website if questions arise.
• Fees should be expected as reports, letters take valuable Physician and Staff time.
